The Key to Effective Case Management for Youth: Linking Assessment to Service Delivery

This webinar will bring together YouthBuild and Workforce Investment Act (WIA) Youth providers for valuable peer sharing across the youth services community. Webinar participants will have the opportunity to ask questions and participate in an online chat. Case management is a client-centered, goal-oriented strategy for providing assessment-based services to meet individual and program goals. By utilizing strong case management strategies, case management services can be tailored to anticipate and proactively address the needs of youth. Thoughtful delivery of services empowers youth to meet their goals and achieve self-sufficiency.

  • Constructing Individual Service Strategies (ISS) based on assessed needs;
  • Planning and implementing services for a young person; and
  • Providing regular, updated case notes that accurately describe each youth’s progression.
